The condition of graded products is open for interpretation by suppliers within the trade. You do have to be very careful when purchasing graded goods and ensure that you are aware of exactly what you are getting for your money. As others have suggested, a key or suppliers grade guide to what they class as different grades is usually provided or can be requested. Do make sure that you ask if they are not being transparent in this regard.

As a general rule of thumb:

Grade A = Usually as new, may not come in original box or with manufacturers accessories/warranty, sold as working
Grade B = Usually has signs of wear and tear, perhaps even used or ex-display models, expect the odd unit to be worse than others, perhaps even some that do not work
Grade C = Take your chances, usually heavily worn/used, a large percentage will contain major defects and require repair or replacement parts before sold at consumer level

Go steady with what you purchase and never walk into a transaction without clarifying exactly what it is you are getting for your money.
